At the Alliance for Positive Health, we work with those who face barriers to accessing health care and social services and strive to reduce health disparities. Our clients’ diagnoses are often complicated by stigma, addiction, poverty, homelessness, incarceration or mental illness – and these are the factors which also place them at risk of infection with HIV, Hepatitis C and other Sexually Transmitted Diseases.
We provide care coordination for people with chronic diseases including HIV/AIDS, diabetes, hypertension, heart disease, obesity, serious mental illness and asthma. We work in partnership with our clients to help them navigate complex systems. We provide direct assistance and skills building to help them become more independent and successful in their disease management.
We prevent new cases of HIV, Hepatitis C and Sexually Transmitted Infections (STIs) by reaching those who have the highest chance of exposure, raising awareness of personal risk, conducting thousands of HIV, STIs and Hepatitis C tests each year, and reducing the stigma and discrimination which continue to surround these diseases and the people who are most impacted by them. We believe information and advocacy help clients better access health related services and enables them to make informed, healthier decisions.
